Hi Hui Li, thank you for reading the case study and for reaching out to me.

This is quite common for patients with antalgic gait to revert back under certain conditions such as an increase in physical demands on the back.

There are a few factors here that might be beyond your control:
1. Do the patient actually have a bulging disc or a herniated disc? - Usually the antalgic gait is a direct response to a major structural impairment such as a herniated disc - and if it is the case, some sort of a surgical procedure like microdiscectomy might be warranted here before we can actually permanently correct it.

2. How committed and engaged is the patient? - The guy in our case study was very determined to resolve his back issue no matter what. He came in emotionally broken about it, but as he started seeing results his commitment grew x10 fold.
He also came to see us after already getting a microdiscectomy procedure a few months prior - which might have allowed us more effect with needling the muscles around the region.

3. How many visits did he get and for how long ? Our guy was coming in for care for 3-4 months. He initially started with 2x/week for 6 weeks and then 1-2x/week for another 6-10 weeks.
Lastly and perhaps most importantly, as his pain started to diminish with the acupuncture, he started focusing on strengthening his back muscles. From doing daily corrective exercises regime we prescribed him to swimming 3-4 times a week and then to being fully engaged in the back strengthening program called &quot;Foundation Training&quot; (www.FoundationTraining.com) by Dr Eric Goodman - which I highly recommend here as well.

All of these together along with the non-compromising attitude to resolve this issue were all playing a big part in his recovery.

If you haven&#039;t done that yet, Hui Li, I would highly recommend doing the SMAC program by Matt Callison and Brian Lau to really get more specific details about useful needling techniques and which corrective exercises to apply to go along with it.
